Statewide HACC Equity & Access Project

ADEC’s Statewide Equity and Access Project has a leading role in supporting over five hundred Home and Community Care (HACC) providers to implement cultural action plans to improve culturally responsive services throughout the State. The project is a part time position of 23 hours, a member of the ADEC Equity and Access Team and is funded through the HACC System Resourcing service type.

Cultural Action Plans aim to improve services to HACC carers and consumers from ethnic communities experiencing frailty and/or disability. The statewide project assists regions to implement action plans across Victoria in partnership with DHS and eleven other Equity and Access projects based in different parts of the state. Activities include coordination, peer support, orientation of new workers, sharing of skills and resources, reducing isolation, assisting with training sessions, improving assessment and developing common strategies to improve implementation.

The Equity and Access Forums

The State-wide Equity and Access project also supports other projects with training, review and information through regular state-wide forums. Four statewide forums are convened annually at ADEC for over thirty Equity and Access projects in Victoria (see Equity and Access Forums for details and dates).
